Determination of Human Subjects Review Checklist
This guide is to be used by WSU Extension personnel to determine if a research or evaluation activity should be reviewed by the WSU Institutional Review Board (IRB). All Extension personnel must comply with the policies and procedures of the IRB manual (Institutional Review Board).
Before you start, check with the author or originator of the survey to see if an IRB approval currently exists for the process or survey that you plan to use. For example, the Life Skills evaluation that many 4-H educators use has been previously approved by IRB. These assessments can be conducted under this approval.
