Life Skills Evaluation System

LIFE SKILLS, JUNIOR - STEP FOUR: CREATE EVALUATION FORM

A. Log In
Log in by entering your WSU Network ID (NID) and password. Washington State University employees and students can create their Network ID if they do not have one already. If you do not have a WSU ID number and therefore cannot create a WSU Network ID, contact Computing and Web Resources Support at cwr.support@wsu.edu or 509/335-2813.

Once you have logged in, you are now in the Life Skills Evaluation System.

B. Create form
  1. Click on the Create Evaluation Form button to create an evaluation form.
  2. A window will appear asking you if you have completed the necessary procedures for participants under 18 years of age. Click the appropriate button to answer. If you have NOT notified the parents/guardians of program participants, the system will take you back to the Notice of Evaluation page so you can read about what you must do.

    Select an age range for participants. This determines which component will be used: Life Skills, Senior or Life Skills, Junior.

    Click Next to continue.
  3. Enter Program Information
    Type the name of the educational program you are evaluating in the text box. Then select the Delivery Method from the drop-down menu.
  4. Select Outcome Indicators
    Select 5-10 indicators to be included in your evaluation form from the list of indicators. Choose indicators based on your program's outcomes. Click in the box next to an indicator to select it.
  5. Create Additional Indicators
    Below the "Outcome Indicators" there are 10 text boxes in which you can add your own program indicators. This is optional but may be useful for your local program.
  6. Review Your Choices
    Click Next at the bottom of the page. This will bring up a review page of your evaluation choices thus far so you can review what you have chosen and make changes, if necessary. Click Previous if you want to make changes. If everything is correct, click on Finish to save your evaluation form.

    Your evaluation form is saved for you within the Life Skills Evaluation System as a Rich Text (rtf) document and can be accessed at any time through this Web site.


C. View Actual Form
To view your form, click "View Evaluation Form" from the menu on the left. Follow the directions given on screen. Save a copy of the form in a file on your local computer. You can now add any graphics or logos to personalize the evaluation form, but do not remove the WSU Extension logo or the Consent paragraph at the top.

You will notice at least three census-type questions (pre-test) or open-ended questions (post-test) at the end of your form. You can input participant responses to these questions into the Evaluation System. When the report is printed, these responses will print out as you have entered them.

D. Print Copies
Print copies of your evaluation form to distribute to program participants. Have participants complete the (post-test) evaluations during one of the last sessions of your program.
