  Why Evaluate?

STEP FOUR

Log In
Log in by typing "cahe\" and your WSU user name and password. If you do not have a WSU user name or are unsure what yours is, contact Tony Wright at wright@wsu.edu or 509/335-2814.

Once you have logged in, you are now in the Evaluation System.

Participant Consent
Click on the Create button to create an evaluation form. A window will appear asking you if you have completed the necessary procedures for participants under 18 years of age. Select the correct button & click "Continue." If you haven't obtained the proper permissions, the system will take you back to the Participant Consent page.

Enter Program Information
The next window will ask you to identify the name of the educational program you are evaluating, the Delivery Method, and the Plan of Work area this program fits into, if any.

Select Outcome Indicators
Select 5-10 indicators you want on your evaluation form from the list of life skills outcome indicators. Select indicators based on your program's objectives and goals. Click in the box next to each indicator you select.

Select Optional Indicators
At the end of the list of "Life Skill Outcome Indicators" there are 10 blank boxes you can use to add your own program indicators. These indicators are optional, and data collected on them will not be entered into the Evaluation System, but may be useful to gather for your local program. If you are going to publish your evaluation results in the local newspaper or anywhere else, you must have the optional indicators approved by the WSU Institutional Review Board. See Participant Consent.

Create Your Form
At the bottom of the page click on Build It.

After you have clicked on Build It, you will see the information you have input in the first window. This gives you a chance to review what you have done and make changes, if necessary. Click on Go Back if you want to make changes. Click on Accept if everything is correct. Your evaluation form has now been saved.

View & Personalize Your Form
To view your form as an RTF, click "Generate an RTF File" on the left. Follow the directions given on screen. Save a copy of the form in a file on your computer. You can now add any graphics or logos to personalize the evaluation form, but do not remove the WSU Cooperative Extension logo.

You will notice three open-ended questions at the end of your form. This information is for your local use only and will not be entered into the Evaluation System.

Print Copies
Print copies of your evaluation form to distribute to program participants to complete during one of the last sessions of your program.

Using the Form
The outcome indicators listed on this Web site (View Life Skills) have been written to provide useful information and yet be general enough to apply to various Family Living and 4-H Youth Development programs. Therefore, when administering the evaluation you will need to instruct the participants to think about the statements in relation to the program they have just completed. You might say, " Think back on what you have learned in this (name of your program) on (topic of the program). Respond to the statements based on this program."
