LIFE SKILLS, JUNIOR - STEP THREE: NOTICE OF EVALUATION

The WSU Extension Life Skills Evaluation System has been reviewed by the WSU Office of Research Assurances
(ORA). The ORA has determined that the Life Skills System is a program evaluation tool that is
conducted to determine the effectiveness of our programs. Therefore there is no legal mandate to require
written consent from our adult participants or parental consent from the
parents/guardians of our youth participants. HOWEVER, because we want to be ethical, open and truthful,
in dealing with all the participants in our programs WSUE requires that:

A written notice must be provided to all parents/guardians whose children under 18 years of age may be
asked to participate in the evaluation of your program. (See notice below.) We suggest that you
send a copy of this notice along with your program materials when youth enter your program. You
only need to give the notice to parents once a year. It is then their responsibility to contact
you if they do not want their child to participate in the evaluation. If you do not hear from the
parents, then their son or daughter can participate, if the youth so desires.

The youth in your program must be informed about the evaluation. For this reason the paragraph at the
top of the customized evaluation form that explains the participants' rights in completing the
evaluation must remain on your evaluation form. You need to read it with the participants (both
youth and adult) who will be completing the evaluation.

Notice to Parents/Guardians
As a participant in the
(name of the program) your son or daughter may be asked to help with the
evaluation of the program. At the end of each program or program year, we conduct an evaluation to tell
us how well the program is working. Your son or daughter may be asked to complete a written survey about
what he or she may have learned from participating in the program. We estimate that it will take the youth
participants approximately 10 minutes to complete the survey.
Youths are not required to participate in the evaluation. If your son or daughter decides that he or she
does not wish to participate, it will not affect his or her participation in this or future WSU Extension
programs. If your son or daughter does not want to answer some questions on the survey, that is okay. The
survey responses will be anonymous, and your son's or daughter's responses will not be identified in any
way.
